作用机制
血管紧张素 II（由血管紧张素 I 在血管紧张素转换酶 ACE / kininase II 的催化下生成）是一种强力的血管收缩剂，是肾素-血管紧张素系统中的主要活性激素，也是高血压发病机制中的重要因素。同时，它还能刺激肾上腺皮质分泌醛固酮。
氯沙坦及其主要活性代谢物通过选择性阻断血管紧张素 II 与 AT1 受体的结合，从而抑制血管紧张素 II 的血管收缩效应和刺激醛固酮分泌的作用。AT1 受体存在于多种组织中（如血管平滑肌、肾上腺）。尽管 AT2 受体也存在于多种组织中，但目前尚未发现其与心血管稳态调节有关。
氯沙坦及其主要活性代谢物在 AT1 受体上均不具有部分激动活性，并且与 AT1 受体的亲和力比 AT2 受体高约 1000 倍。体外结合研究表明，氯沙坦是 AT1 受体的可逆性竞争性抑制剂；而其活性代谢物的效力比氯沙坦高 10–40 倍，且似乎是 AT1 受体的可逆性非竞争性抑制剂。
氯沙坦及其活性代谢物均 不抑制 ACE（将血管紧张素 I 转化为血管紧张素 II 的酶，同时降解激肽），也 不与其他对心血管调节重要的激素受体或离子通道结合或产生阻断作用。
适应症
可沙星（COZAAR）是一种血管紧张素II受体阻滞剂（ARB），适用于：
- 高血压治疗：适用于成人及6 岁以上儿童，用于降低血压。降低血压可减少致死性和非致死性心血管事件的风险，主要包括中风和心肌梗死。
- 降低中风风险：适用于高血压合并左心室肥厚的患者。研究显示，这一益处在黑人患者中可能不适用。
- 糖尿病肾病治疗：适用于2 型糖尿病患者，伴有血清肌酐升高和蛋白尿，同时有高血压病史。
